The all-inclusive option for families who want full access
Membership is a premium, concierge-style primary care experience for families who want more than visits – they want a relationship, direct access, and steady guidance through the early years.
This is for you if:
- You want to text or call Dr. Rosario between visits.
- You want well visits, sick visits and Telehealth all included.
- You want home visits included (up to age 20
- you want a proactive, preventative approach – not just reactive care.
- You loved your newborn concierge experience and want that same elvel of support to continue.
What is included in the membership
Included
- Primary care with a small-panel, relationship-based approach
- Age-appropriate well visits and developmental guidance
- Vaccines and screenings
- Access to after-hours care
- Sick visits and Telehealth included
- Care coordination and referrals when specialty care is needed
- A proactive focus on wellness and foundational health (sleep, nutrition, routines, environment, resilience)
- Home visits included up to age 2.
Not Included
Lab and imaging fees (billed separately through third parties)
Specialist care (GI, Allergy, Derm, Pulm, etc.)
Urgent care/ER/hospital services
Complex care outside scope or conditions requiring specialty management

We keep our panel intentionally small so your visits feel unhurried, your questions are welcome, and the details of your child’s story actually matter.

When something feels off, you have a physician who knows your child and can help you decide what to watch, what to try, and when to escalate.

The early years shape so much – sleep, feeding, digestion, skin, immunity, development. We focus on foundations because they compound over time, and because many small issues become big without early guidance.

How this works
Because the newborn period is high-touch season, babies begin with a Newborn Care Plan that includes 5 planned visits along with closer follow-up and frequent communication in the early weeks. Children older than 2 months may start directly with membership.
- Membership is for children age 0 months to 21 years.
- 3 month minimum commitment if paying monthly
- $150 enrollment fee

7% discount, paid at enrollment and at month 6.
Single Payment

Sibling discount:
• Each additional sibling: $100/month

Foundational Family Special pricing (limited to 15 families)

We do not bill insurance for membership services. Superbills are available upon request for families seeking out-of-network reimbursement (coverage varies by plan)
Primary Care Pay-As-You-Go
No membership required, pay for services as you need them.

- Well child visits with extended time for questions and integrative counseling.
- Sick visits and urgent concerns
- Vaccines (costs vary depending on the vaccines given, no admin charge)
- Care coordination, referrals and follow-up messages related to recent visits
Visit fees:
- New patient visit: $450
- Established visit: $350
- Home visit: $150 add-on travel fee
- Telehealth (established patients only): $250
- Telehealth (weekend/holiday): $350
- After hours or weekend communication (phone/text): $25
No monthly fees. You only pay when you schedule a visit. Superbills available for possible out-of-network reimbursement (varies by plan).
